29 CFR 1910.67(c)(2)(ii): The employer did not ensure that only trained persons operated an aerial lift:     a) Jobsite, 400 Settlers Ridge Center Road, Pittsburgh, PA: On or about December 20, 2012, during light bulb replacement and vent cleaning activities, the employer did not ensure that only properly trained employees operated a Genie model Z-30/20N aerial boom lift. An employee was working at a height determined to be approximately 40 feet in the air and was not using any fall restraint or fall arrest equipment.

29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:   a) Jobsite, 400 Settlers Ridge Center Road, Pittsburgh, PA: On or about December 20, 2012, during light bulb replacement and vent cleaning activities, the employer did not ensure that an employee utilized appropriate fall protection systems/devices while operating a Genie model Z-30/20N aerial boom lift. An employee was working at a height determined to be approximately 40 feet in the air and was not using any fall restraint or fall arrest equipment.

The cord had damaged outer jacket areas and was covered in at least one place with duct tape over a cut/tear in the outer insulation jacket.
