29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     a)  Employees were exposed to fall hazards while performing roofing work and the employer did not provide a training program to recognize fall hazards and the procedures to be followed in order to minimize those hazards.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    a)  Employees engaged in roofing activities on a low slope roof were exposed to a fall hazard of approximately 18.6 feet.     Domain Corporation, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10) which was contained in OSHA violation number 313591398, citation 1, item 1, issued on August 4, 2009, and became final order on or about August 25, 2009, with respect to a workplace located at 1887-1807 Chestnut/Admiral Ct., Glenview, IL 60026, and OSHA violation number 313931594, citation 1, item 1, issued on January 22, 2010, and became final order on or about February 17, 2010, with respect to a workplace located at 8842 Ogden Ave., Brookfield, IL 60513.    No abatement certification or documentation is required for this item.
