29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Gilson Machine and Tool, Inc., Napoleon, Ohio: On or about December 27, 2012, the employer failed to develop and implement a lockout program/procedures and training to protect employees from injuries while they engaged in servicing and maintenance activities. Employees were exposed while performing tasks such as, but not limited to job setups and tooling changes on various CNC machining equipment, exposing them to laceration, crushing and amputation hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Gilson Machine and Tool, Inc., Napoleon, Ohio: On or about December 26, 2012, employees operating the Okuma (#340, Model MX-45UAE, Serial #09109124) CNC vertical milling machine were exposed to injuries from the machine's rotating parts and flying chips, in that the interlock on the sliding door guard was bypassed. Employees were able to operate the equipment with the door open. b) Gilson Machine and Tool, Inc., Napoleon, Ohio: On or about December 27, 2012, employees operating the Okuma (#350, Crown V, Serial #0043) CNC vertical milling machine were exposed to injuries from the machine's rotating parts and flying chips, in that the interlock on the sliding door guard was bypassed. Employees were able to operate the equipment with the door open.
