Specifically, an employee was witnessed and photographed leaning over the edge of the flat roof to perform some roofing work. The warning line system used was down, less than 6 feet from the edges and a safety monitor was not used. Condition exposed employee to fall hazards of approximately 25 feet.    Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
