29 CFR 1910.95(g)(8)(ii)(B): Unless a physician determines that the standard threshold shift is not work-related or aggravated by occupational noise exposure, the employer did not ensure, when a standard threshold shift occurs, that for employees already using hearing protectors, employees be refitted and retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation if necessary: (a) On or about 5/2/13, the gun puffer was exposed to 87.3 dBA as an 8-hr time-weighted average (TWA). In addition, he was listed on the 2012 OSHA Form 300 as having a standard threshold shift on 6/22/12. He had not been effectively refitted and retrained in the use of hearing protectors in accordance with this subpart.

In addition, he was listed on the 2012 OSHA Form 300 as having a standard threshold shift on 6/22/12. The employer did not ensure that he wore hearing protectors, in accordance with this subpart.
