1910.26(c)(3)(iv):  The top of the ladder was not placed with the two rails supported, unless equipped with a single support attachment:      Jobsite:  An employee fell from an extension ladder with a single support attachment (stabilizer) that was not installed according to manufacturer specifications.    On or about 28 December 2012 an employee was injured from a fall while working from an aluminum extension ladder that was set up on a porch.  The ladder had a stabilizer attached that was installed improperly causing the stabilizer to tilt to one side.  The employee fell off the ladder hitting the railing of the deck and was taken to the hospital with bruises.
