Employees were exposed to head injuries from being struck by falling masonry materials from the upper tiers of the scaffold as some workers were not wearing hard hats. Employees were exposed to falls of up to 26 feet to the ground below from the tubular welded frame scaffold's upper platforms as the scaffold planks had more than a 1 inch wide space between some adjacent units.
