Admiral Glass & Mirror failed to complete and send to OSHA, or its agent, the "OSHA Occupational Injury and Illness Data Collection Form, 2011" within 30 days of the date Elmore Interest Inc. Admiral Glass & Mirror received the Form, which was mailed to Elmore Interest Inc. Admiral Glass & Mirror in June 2012. Elmore Interest Inc.
