1910.23(a)(6): Every manhole floor opening shall be guarded by a standard manhole cover which need not be hinged in place. While the cover is not in place, the manhole opening shall be constantly attended by someone or shall be protected by removable standard railings. a) Car Wash Express at 2295 East 120th Avenue, North Glenn, CO 80234: On and before 12/28/12, the employer did not ensure that each manhole cover was attended to or protected by a standard railing. This condition exposed employees to a tripping and fall hazard. Abatement Note: Abatement certification is required for this item (See enclosed Sample Abatement-Certification Letter).

29 CFR 1910.101(b): Section 3.3.1, Compressed Gas Association Pamphlet P1 1965, as adopted by 29 CFR 1910.101(b): Compressed gas cylinder(s) should be stored in accordance with all local, state and municipal regulations and in accordance with appropriate standards of the Compressed Gas Association and the National Fire Protection Association. (a) Car Wash Express at 2295 East 120th Avenue, North Glenn, CO 80234: On and before 12/28/12, the employer stored oxygen compressed gas cylinders and acetylene cylinders together in the maintenance shop. Section 3.5.3 of Pamphlet P-1 provides that stored oxygen and fuel gas cylinders should be separated by a minimum of 20 feet, or there should be a fire resistant partition between the oxygen and fuel gas cylinders. Abatement Note: Abatement certification is required for this item (See enclosed Sample Abatement-Certification Letter).

All-purpose cleaning solution was transferred into smaller containers, hydraulic fluid and used oil drums were not labeled. This condition exposed employees to a chemical hazard. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
