1910.23(a)(6): Every manhole floor opening shall be guarded by a standard manhole cover which need not be hinged in place. While the cover is not in place, the manhole opening shall be constantly attended by someone or shall be protected by removable standard railings.    a)	Car Wash Express at 2295 East 120th Avenue, North Glenn, CO 80234: On and before 12/28/12, the employer did not ensure that each manhole cover was attended to or protected by a standard railing. This condition exposed employees to a tripping and fall hazard.      Abatement Note:  Abatement certification is required for this item (See enclosed Sample Abatement-Certification Letter).

29 CFR 1910.1200(f)(5)(i): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the hazardous chemical(s) contained therein:     a)	Car Wash Express at 2295 East 120th Avenue, North Glenn, CO 80234: On and before 12/28/12, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the hazardous chemicals contained within.  All-purpose cleaning solution was transferred into smaller containers, hydraulic fluid and used oil drums were not labeled.  This condition exposed employees to a chemical hazard.         Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
