29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a)Central Church Road Baptist Church: On December 28, 2012, four employees were exposed to fall hazards of approximately 30 feet while replacing roof shingles without any fall protection on a church. The employer did not ensure that fall protection measures were in place to protect employees from falling. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper level landing surface and the ladder length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder. a)Central Church Road Baptist Church: On December 28, 2012, portable ladders being used by employees were not placed so that the ladder side rails were at least 3 feet above the surface level, exposing employees to a fall hazard. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED.
