29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):    (a) At the St. Charles Rock Road work site frequent and regular inspections were not conducted as evidenced by employees:              1. Working at a height of 20 feet at the edge of the building roof without the use of fall protection       2. Using an extension ladder which had not been properly extended at least 3 feet  beyond the roof edge to access the roof    Abatement verification is required for this violation.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:     (a)At the 9216 St Charles Rock Road site, 2 employees were exposed to a fall hazard in that the employees were  working at the unprotected edge of a roof approximately 20 feet above ground without benefit of fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    (a) At the St. Charles Rock Road work site employees had not been trained about hazard recognition pertaining to fall hazards and the procedures to be followed in order to minimize these hazards:              1. Working at a height of 20 feet at the edge of the building roof without the use of fall protection       2. 29 CFR 1926.1053(b)(1):  When portable ladders were used for access to an upper landing surface, and the ladder's length allowed, the ladder's side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a)At the 9216 St Charles Rock road site, two employees were working on a building repairing roofing edging.  The two employees accessed the roof of the building using an extension ladder which did not  extend at least 3 feet above the roof edge exposing employees to a fall of approximately 20feet.
