29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system; nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about November 30, 2012, at a job site located at 4450 N. Sienna Ridge in Riverside, MO, a worker was exposed to a fall hazard of greater than 25 feet in that the worker was observed to be standing on a rafter beam protruding outside the external wall of the building. No form of fall protection system or equipment was in place or in use at the time of the inspection. Further, workers were exposed to interior fall hazards of greater than 6 feet due to unguarded openings existing in the second story floor decking (e.g., stairway openings).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) On or about November 30, 2012, at a job site located at 4450 N Sienna Ridge in Riverside, MO, workers were exposed to serious fall hazards in that they had not been trained in fall hazard recognition and the proper installation or use of the equipment, procedures and systems designed to mitigate those hazards.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge: (a) On or about November 30, 2012, at a job site located at 4450 N Sienna Ridge in Riverside, MO, workers were exposed to serious slip, trip and fall hazards while using stairways from the basement to the first floor and from the first floor to the secound. The stairways lacked handrails and no stairrail systems were in place on unprotected sides.
