(b) Custom Carbon Processing site:  On or about December 29, 2012, and at times prior thereto, the employer did not take precautions to prevent ignition sources such as, but not limited to space heaters in flammable areas. Non-intrinsically safe equipment was used in a flammable area. Employees exposed to explosion and/or fire from a diesel powered heater located near the office area.
