Unqualified employees had access to the circuit breaker panel to turn on and off unknown machines and lights, creating unexpected energization.  Employees were exposed to electrical shock hazards, and burns.

Employees were exposed to electrical shock and fire hazards.     a)  240 volt 3 phase panel box on the southeast wall had the working space blocked by an wooden pallet and cardboard boxes of material and a wooden pallet and pallet jack.      b)  A live fuse and relay panel box on the Southeast wall had the working space blocked by a wooden pallet and cardboard boxes of equipment and material.

29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and/or cables were run through holes in walls, ceilings, or floors:    On or about December 31, 2012 and at times prior thereto, at the security station where employees enter and exit the warehouse, flexible cords were run through the ceiling exposing employees to fire hazards.    a) Along the side wall, an extension cord was plugged into the wall outlet and run through the ceiling tile.    b) Above the security scanner, the scanner flexible cord was run through the ceiling tile.
