29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:     (a)  In the mechanic shop above the offices, the railing was not equipped with a midrail that ran continuously across the storage mezzanine. Employees were exposed to fall hazards when accessing the area to retrieve rags.

29 CFR 1910.27(d)(3):     The employer did not ensure that the side rails of a through ladder extended 3 1/2 feet above parapets and landings, omitting the rungs from the extension and having not less than 18 nor more than 24 inches clearance between the rails.     (a)  In the mechanic shop, the side rails of the fixed ladder did not extend above the landing. Employee(s) were exposed to at least 8-foot fall hazards when using the ladder to access the storage mezzanine.
