The employee was working in the area and walked over a flexible cord used to energize a computer that was in a pinch point, between the door and door frame.

29 CFR 1910.157(c)(1): The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury.    a.  The fire extinguisher near the loading bay doors in the warehouse was blocked and not readily accessible for employees in the event of emergency.
