Instance a.  Site Service Bay - On or about 1/3/2012, employees were operating the following: 1) Eagle Equipment (Maximizer model# MX9C S/N# MX9C-0601-FNRMB-017)  two post swing arm lift, which did not have an annual inspection performed on it by a qualified person.    Instance b.  Site Service Bay - On or about 1/3/2012, employees were operating the following: 1) Eagle Equipment (Maximizer model# MX9C S/N# MX9C-0601-FNRMY-018)  two post swing arm lift, which did not have an annual inspection performed on it by a qualified person.
