29 CFR 1910.23(a)(1): Every stairway floor opening was not guarded by a standard railing constructed in accordance with paragraph (e) of this section.    On October 30, 2012, and times prior to, trucking contractor employees were required to use a catwalk at Western Refining asphalt loading station which was not properly guarded by standard railing on all open sides.  This condition exposed employees to fall up to 13 feet to the ground.
