29 CFR 1926.501(b)(1):  "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a. On 1-4-2013 an employee kneeling on the top of a three foot parapet 28-30 feet above a paved sidewalk was exposed to a fall hazard.    b. An employee on a mezanine approximately 28-30 above a paved sidewalk without fall protection where he was observed working on removing snow and ice.

29 CFR 1926.1053(b)(7): Ladders shall not be used on slippery surfaces unless secured or provided with slip-resistant feet to prevent accidental displacement. Slip-resistant feet shall not be used as a substitute for care in placing, lashing, or holding a ladder that is used upon slippery surfaces including, but not limited to, flat metal or concrete surfaces that are constructed so they cannot be prevented from becoming slippery.    a. Employees were using ladders on the rooftop of Krogers on packed down snow, without securing the ladder to ensure stability.
