29 CFR 1910.252(a)(2)(iii)(A)(1): The employer did not provide fire watchers whenever welding or cutting was performed, where appreciable combustible material in building construction or contents was closer than 35 feet (10.7 m) to the point of operation: (a) Universal Stainless Facility, Creek Building, Bridgeville, PA: On or about 12/19/12 a fire watcher was not provided when cutting operations were conducted approximately 18 feet above the plate leveler. A fire occurred when a spark or hot material fell onto the unprotected hydraulic hoses lying on the floor. The fire was extinguished by an employee with the use of a fire extinguisher obtained from one of the host employer's fork trucks that was parked in the area.

A fire occurred when sparks and/or hot, cut material(s) fell onto uncovered hydraulic hoses lying on the floor below the work area.

The cutting operations had been conducted without having fire extinguishing equipment readily available for use. The supervisor utilized fire extinguishing equipment found on the host employer's fork truck that was parked in the area to extinguish the fire.
