29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end. (a)Front side, new residential construction located at 617 Concord Circle, Smyrna, GA 30080: Employees were exposed to fall hazards when working on a 30' aluminum platform that extended 9' past the support post with no guardrail system to block employees access to the cantilevered end. The work platform was located at heights of approximately 18'11" from the ground.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. (a) Front side, new residential construction located at 617 Concord Circle, Smyrna, GA 30080. Employees were exposed to fall hazards of approximately 18'11" to the ground while installing gable vents, rake boards, and soffit on a 30' aluminum scaffold plank which was not provided with mid, side, and top guard rails along all open sides.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. (a) Front side, new residential construction located at 617 Concord Circle, Smyrna, GA 30080: Employees were exposed to fall hazards of 18'11" when makeshift bracing was used to secure an Alum-A-Pole Pump Jack Scaffold System. The employer failed to use the rigid triangular bracing manufactured for this system when erecting the scaffold.
