29 CFR 1910.303(f)(2):     Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:(a) (LOCATION) (IDENTIFY EQUIPMENT) (STATE HAZARD(S) TO EMPLOYEE(S))    The employer failed to protect employees from potential electrical hazards associated with not having circuit breakers marked in panel boxes.  This was most recently evident during the inspection of the facility located at 13075 Renfro Circle Omaha, NE 68137, where the employer has employees accessing an unlabeled electrical panel box to turn off power to various overhead lighting fixtures and pieces of equipment.

29 CFR 1910.305(b)(1)(ii):     Unused openings in boxes, cabinets, or fittings were not effectively closed:(a) (LOCATION) (IDENTIFY EQUIPMENT AND CONDITION(S)) (STATE HAZARD(S) TO EMPLOYEE(S))    The employer failed to protect employees from potential electrical hazards associated with employees accessing electrical panel boxes with unused openings.  This was most recently evident during the inspection of the facility located at 13075 Renfro Circle Omaha, NE 68137, where the employer has employees accessing electrical panel boxes with unused openings to turn off power to various lighting fixtures and pieces of equipment.  Instances include but are not limited to:    i.  Employees accessing C-01 electrical panel box near the receiving dock to turn off the power to the overhead door and nearby compressor and there is a knockout missing from the box.    ii.  Employees accessing the electrical panel box near the combo room to turn off power to the overhead lighting and there are two circuit breakers missing and no blank adapters installed.

29 CFR 1910.134(c)(2)(i):     Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: (Construction Reference 1926.103)    The employer failed to protect employees from potential respiratory hazards associated with not providing exposed employees with the information required per Appendix D of this subpart.  This was most recently evident during the inspection of the facility located at 13075 Renfro Circle Omaha, NE 68137, where the employer has employees using N95 respirators on a voluntary basis as they clean the facility without having covered with them all the requirements as outlined in Appendix D of this subpart, whereby explaining the hazards associated with wearing a respirator.
