29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    On or about 1/4/2013, an employee was painting steel beams on the 3rd level approximately 26' above ground level inside a commercial property whereas he was not protected from fall hazards.      STEPHENS & SONS PAINTING, INC WAS PREVIOUSLY CITED FOR A VIOLATION FOR THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(B)(1) WHICH WAS CONTAINED IN OSHA INSPECTION  NUMBER 312153356 CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON 2/17/2009 WITH RESPECT TO A WORKPLACE LOCATION AT 1905 OKEECHOBEE BLVD, WEST PALM BEACH, FL 33409.
