29 CFR 1910.133(a)(3): The employer did not ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design, or wears eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses:    On or about  January 4, 2013; in the Oasis CNC department, an employee(s) was wearing perscription glasses and was exposed to flying particles while using pneumatic air regulated at 30 PSI to clean and inspect products for quality exposing employees to eye injuries.

29 CFR 1910.305(g)(1)(iv)(B): Electrical cords were run through holes in walls, ceilings, and/or floors:    On or about 4 January, 2013; in the maintenance shop, an electrical cord was run through a wall in order to power a work light for a Hardinge industrial metal lathe exposing employees to an electrical shock of 115 V.
