29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition On or about December 21, 2012: Employees operated powered industrial trucks that were in need of repair: a) Clarklift Model 500 S100 had a defective back up alarm. b) Clarklift Model C500 Y50 had a defective back up alarm. Employees were exposed to the hazard of being crushed by a forklift when forklift was in reverse.
