On or about January 4, 2013 - at the above addressed jobsite, an employee performing electrical maintenance to a display sign from a SKYTEL Vehicle-mounted Telescopic Boom Aerial Lift basket did not have a body belt and a lanyard secured to the basket, thereby exposing the employee to the hazards associated with a fall of approximately five(5) feet five (5) inches.
