29 CFR 1910.303(g)(2)(ii):  In locations where electric equipment was likely to be exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage:    a) Facility, 300 Brush Creek Road, Warrendale, PA:  On or about December 14, 2012, an electrical conduit containing energized 480 v AC electrical wiring was not protected against damage.  An employee operating a powered industrial truck hit and sheared off the conduit and shorted out this wiring.  This condition exposed employees to a potential electrical shock hazard.
