29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite: On or about 01/07/2013 at the 850 Silas Deane Highway re-roofing project, employees were applying flammable roofing adhesives to the roof deck with no fire extinguishers on the roof.

29 CFR 1926.502(f)(2)(ii): The rope, wire, or chain was not rigged or supported in such a way that its lowest point (including sag) was not less than 34 inches (0.9 m) from the walking/working surface and its highest point no more than 39 inches (1.0 m) from the walking/working surface. On or about 01/07/2013 at the 850 Silas Deane Highway re-roofing project, employees were working on the roof deck installing EPDM roof membrane and the Warning Line System was lying on the roof deck.
