29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: On or about December 31, 2012 in the pit at the grain elevator a sump pump was energized by a blue SJTW extension cord. The grounding prong was missing from the cord. This condition created an electrical hazard to employees using the pump to pump out rain water from the sump.
