29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition: On or about January 2, 2013 the left side of a seat belt on a TCM forklift was missing. This condition exposed the operator to the hazards of being struck by the forklift and /or being ejected in the event of an accident in the loading dock area.

29 CFR 1910.305(b)(2): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: On or about January 2, 2013 the cover on a 110 VAC electrical outlet was missing. The outlet was located in the feed mill machine area and created an electrical hazard to workers.
