29 CFR 1910.179(j)(2)(iv): Monthly inspections of hoist chains, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected, were not performed: On or about January 3, 2013, a hoist used to lift down animals during unloading did not have inspection records. The hoist was located in the unloading area and created a struck by hazard to employees.
