29 CFR 1910.215(a)(4):  Work rests on grinding machinery were not adjusted closely to the wheel with a maximum opening of one-eighth inch:    On or about January 9, 2013, employees were exposed to caught-in and struck-by hazards from abrasive wheel grinders with work rests adjusted to more than one-eighth inch from the wheel:    a)  Central Machinery 8" Bench Grinder (#39798) in Manual Area.  b)  Delta Shop Master Bench Grinder in Manual Area.  c)  Central Machinery 8" Bench Grinder on east side of MX Machine 9001.  d)  Central Machinery 8" Bench Grinder (#37823) on east side of MX Machine 9001.
