Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards.     a) Production area: Proper inspection of the Woods Power Grip Manual Tilter, including, but not limited to, inspection of the air filter and performance of a vacuum test, were not conducted as required by the owners manual and ASME Standard B30.20 sections 2.3.1 through 2.3.3, on or about 1/8/13.    b) Production area: Inspections of the Woods Power Grip Manual Tilter were not documented as required by ASME Standard B30-20 section 2.3.6, on or about 1/8/13.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
