29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder(s) was/were not secured at its top to a rigid support, and/or a grasping device, such as a grabrail, was not provided to assist the employees in mounting and dismounting the ladder: This violation most recently occurred on January 7, 2012, at a workplace located at 5201 Collin McKinney Parkway, McKinney, TX; where at least one job made ladder was used to access the first floor were not extended at least three feet above the landing surface.
