The scissor lift was positioned adjacent to the UO2A module.  Module UO2A was suspended from a 40 metric ton overhead bridge crane by four, 10 feet long and 1 and 1/2 inch diameter slings when it struck and toppled the scissor lift sending the two workers in the lift to a 12 feet fall.
