29 CFR 1910.184(c)(9): Employee(s) were not kept clear of load(s) suspended, or about to be lifted by sling(s) (a) Assembly Bay 3, JHSV 633, module UO2A, 03 level, port side - On January 7, 2013 two employees were working from an scissor lift exposed to a suspended load. The scissor lift was positioned adjacent to the UO2A module. Module UO2A was suspended from a 40 metric ton overhead bridge crane by four, 10 feet long and 1 and 1/2 inch diameter slings when it struck and toppled the scissor lift sending the two workers in the lift to a 12 feet fall.

29 CFR 1915.116(j): Loads were suspended over the heads of employees. a) Assembly Bay 3, JHSV 633, module UO2A, 03 level, port side - On January 7, 2013 the operator and his assitant were permitted to work under the level 03 port side module UO2A panel while it was suspended from the crane. The panel was suspended from a 40 metric ton American Crane remote controlled overhead bridge crane.
