29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: A hazardous energy control program had not been established to protect employees from hazards associated with the unexpected energizing of machines. Employees in the blending area were exposed to amputation hazards.

29 CFR 1910.305(d)(2): Panelboard enclosures. Panelboards shall be mounted in cabinets, cutout boxes, or enclosures designed for the purpose and shall be dead front. However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons. On or about January 8, 2013, on the back wall of the facility, an electrical panelbox did not have a cover exposing employees to electrical hazards.
