29 CFR 1926.102(a)(1):     Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.      On or about January 7, 2013, at the jobsite employees were observed not wearing eye protection while using hammers and powered saw to install roofing tiles.  The employees were exposed to eye injury hazards.

29 CFR 1926.451(f)(15):     Ladders were used on small scaffolds to increase the working level height of employees:       On or about January 7, 2013, at the jobsite a portable extensible ladder was observed being used on a small homemade wood platform which was mounted onto a forklift truck and raised to a height greater than 6 feet high.  Employees were exposed to fall hazards.

29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems:      On or about January 7, 2013, at the job site employees were observed installing roofing tiles by the edge of the roof on the flat roof of a building more than 6 feet high to a lower level.  The employees were wearing personal fall arrest system but not tying off.   Employees were exposed to possible fall hazards.

29 CFR 1926.602(c)(1)(ii):     Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval:      On or about January 7, 2013, at the jobsite employees were working from a homemade wood platform which was mounted on an all-terrain forklift truck and raised to a height approximately 27 feet high.  The employer did not have a written approval from the forklift truck manufacturer to allow such addition/modification to the forklift.  The forklift truck or the platform could collapse and injure the employees.
