29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about January 8, 2013, the employer did not ensure employees performing work from a roof were in use of fall protection. The employees were caulking the edges of a metal roof with unprotected sides and edges. The employees were exposed to a fall hazard of more than 6 feet to the concrete ground below.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. On or about January 8, 2013, the employer did not ensure the side rails of an extension ladder extended at least 3 feet above the landing exposing employees to a fall hazard.
