29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:    The 3 metal junction boxes, attached to the main distribution panel located inside the crematorium, were not installed in a neat workmanlike manner.  The boxes were not mounted to a fixed surface nor connected to the conduit.
