(a) On or about 01/08/13, at the Golisano Institute for Sustainability, on the campus of Rochester Institute of Technology (RIT), located at One Lomb Memorial Drive in Rochester, New York, a Ryobi Mitre saw was in use while on the floor and not anchored in place.
