(a) East Jobsite - On or about January 07, 2013 three employees were working below a suspended load. The employees were not wearing hard hats exposing them to struck-by hazards.

(a) Jobsite - On or about January 07, 2013 an employee was using a Brambles scissor lift to install adhesive tape without the end guardrail chains fastened. The employee was exposed to a fall height of 24 feet.

29 CFR 1926.760(a)(1):     Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level shall be protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems.    (a) East Jobsite - On or about January 07, 2013 an employee was on the edge of a 1/12 slope roof staging insulation and sheet metal to be installed on the roof without guardrail system, safety net system or personal fall arrest system. Employee was exposed to a fall height of 17 feet 06 inches.

29 CFR 1926.1053(b)(1):     When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.    (a) East Jobsite - On or about January 07, 2013 an extension ladder being used by employees was not extended at least 3 feet above the landing of the 1/12 pitch roof edge. The roof edge was 17 feet 06 inches above ground.

(a) East Jobsite - On or about January 07, 2013 the 1988 Ford 370-4V boom truck crane was being used to lift insulation and sheet metal roofing did not have information regarding the operation of the crane.
