29 CFR 1910.147(e)(2)(ii): After lockout or tagout devices were removed and before machines or equipment is started, affected employees were not notified that the lockout or tagout device(s) have been removed: On or about December 05, 2012, an employee assisting an electrician during the removal of a jammed part from the Automatic Guide Vehicle (AGV), located in Chassis department #25, was injured while exiting the work area. The employee was not notified that the lockout device(s) were being removed, and hit his head against the hub guard on the AGV as it was being lowered.
