A potential for unexpected startup of the press areas existed when employees set up the slot and printing sections of the press.    c) Skybox Packaging, LLC - Mansfield, Ohio: On or about January 15, 2013, the employer did not assure lockout devices were affixed to the energy isolating device(s) of the #142 S&S Flexo machine before operators perform maintenance activities such as machine set-up and/or cleaning. A potential for unexpected startup of the press areas existed when employees set up the slot and printing sections of the press and cleaning the machine at the end of each shift.    d) Skybox Packaging, LLC - Mansfield, Ohio: On or about January 15, 2013, the employer did not assure lockout devices were affixed to the energy isolating device(s) of the #143 Lian Tiee flexo machine before operators perform maintenance activities such as machine set-up, unjamming, and/or cleaning. A potential for unexpected startup of the press areas existed when employees set up the knives, printing dies, and performing daily machine cleaning activities.     e) Skybox Packaging, LLC - Mansfield, Ohio: On or about January 15, 2013, the employer did not assure lockout devices were affixed to the energy isolating device(s) of the #152 Bobst machine before operators perform maintenance activities such as machine set-up and/or cleaning.  A potential for unexpected startup of the press areas existed when employees set-up and performed cleaning operations.
