The employees were not trained on the recognition of the fall hazard or on the use of harnesses and lanyards.

29 CFR 1926.1053(b)(7): Ladder(s) that were unsecured or not provided with slip-resistant feet were used on slippery surfaces: On or about 1/09/2013 at 6930 Buford Highway Norcross, GA; employees were exposed to a 9' fall to the lower level while performing shingling operations. The 16' extension ladder was rested upon a foam pad to allow it to not slip. There were no slip-resistant feet attached to the ladder. The surface below the pad was asphalt driveway.
