29 CFR 1910.134(k)(5)(ii): Retraining was not conducted when inadequacies in the employee's knowledge or use of the respirator indicated that the employee did not retain the necessary training or skill: On or about January 7th, 2013, employees were exposed to an inhalation hazard while using Azko Nobel Basecoat and other paints containing Xylenes and Benzene in conjunction with having facial hair while wearing a supplied air respirator in the Kayco paint booth.

29 CFR 1910.1200(f)(8): The employer removed or defaced labels on incoming containers of hazardous chemicals and did not immediately mark the containers with the required information: On or about January 7th, 2013, in the draft keg servicing room employees were exposed to a chemical hazard while using Penetrate Beer Line Cleaner, a caustic cleaning agen, that contained Potassium Hydroxide in four unmarked containers.
