29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels: (a) On or about 01/09/2013, for employees working at the shakeout station, cleaning room grinding station, and who operate the Blanchard Grinder. Employer noise exposure records from 03/23/2010 indicated 8-hour time-weighted-average exposures of 88.0 dBA, 94.5 dBA and 87.8 dBA, respectively. Follow-up annual audiometric testing was not completed until 04/30/2013.

29 CFR 1910.213(i)(1): All portions of the saw blade were not enclosed or guarded, except for the working portion of the blade between the bottom of the guide rolls and the table: (a) On or about January 9, 2013, at establishment's Pattern Shop, for employee cutting wood with a Buffalo 14" metal, Model Number WMSAW 3 (EL), wood cutting band saw and also cutting 1/2" plywood with a gray band saw with flood-light directly above cutting area. Saw blades were not guarded at the unused portion of the cutting blade.
