29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: On on about 01/07/2013, at the above mentioned jobsite, employees were working in the warehouse, and materials were stacked on to storage racks and were not secured. PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): On or about 01/07/2013, at the above mentioned jobiste, employees were operating powered industrial trucks that were in need of repair with defects such as missing straps for the propane cylinder, the front lights are damaged and wiring not connected, and right front light not working. a) Komatsu forklift FG15ST-16 - strap is missing on the propane cylinder, front lights are damaged and wiring is not connected to lights. b) Toyota forklift FGCU15 - right front light is not operating.
