Near aluminum and glass storage: The panel box with the black cover had an opening on the side and the top where a knockout was removed and the unused openings had not been effectively closed. 2. First floor office bathroom: The switch for the light in the bathroom did not have a faceplate.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: Near aluminum and glass storage: The outer protective sheath of the flexible cord entering the panel box with the black cover had been pulled back, placing the strain on the wire connections.
