29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. a. On or about January 10, 2013 - at the above addressed jobsite, employees working in a JLG 40H telescopic boom aerial lift were not attached to the boom or basket by the use of a lanyard, thereby exposing the employees to the hazards associated with a fall of approximately six (6) feet.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On or about January 10, 2013- at the above addressed jobsite, an employee performing welding operations on top of a grain dryer, more than twenty five (25) feet above the ground, was not protected from falls, thereby exposing the employee to fall hazards.
