A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. a. On or about January 10, 2013, at the above addressed jobsite, employees conducting water installation in a trench six (6) feet seven (7) inches in depth did not have a safe means of egress. THE AMERICAN UNDERGROUND OF ILL., LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.652(a)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314556713, CITATION NUMBER 1, ITEM NUMBER 2 AND WAS AFFIRMED AS A FINAL ORDER ON JULY 15, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT WILSHIRE AND LILLIAN, ARLINGTON HEIGHTS, IL 60005.

29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a. On or about January 10, 2013, at the above addressed jobsite, employees were conducting water taps in a trench measured at six (6) feet seven (7) inches in depth. The trench was not provided with cave in protection, thereby exposing the employees to cave in hazards. THE AMERICAN UNDERGROUND OF ILL., LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.652(a)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314556713, CITATION NUMBER 1, ITEM NUMBER 3 AND WAS AFFIRMED AS A FINAL ORDER ON JULY 15, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT WILSHIRE AND LILLIAN, ARLINGTON HEIGHTS, IL 60005.
